Find a Health Care Provider Page
Use the Find a Health Care Provider page to find a health care provider by plan, name, type, location, language or gender.
You can use multiple fields below to perform your search. For example, you can select a Provider Type and Gender in one panel, as well as a City and State in another panel, to display a list of providers that match all your search options.
Clicking Search displays the Search Results panel. To see provider details click the Provider ID link to navigate to the Find a Healthcare Provider Detail page.
Plan Name Search Fields

| Plan Name | Indicates managed care organization plan name. If you wish to find providers associated with Managed Care Organization (MCO), please select the appropriate MCO from the dropdown. |
Provider Name Search Fields

| Last Name or Practice Name | Indicates the provider's last name or the name of the practice. Can be used with the Starts With or Phonetic option. |
| First Name | Indicates the provider's first name. Can be used with the Starts With or Phonetic option. |
| Starts With |
The Starts With option tells the system to retrieve all providers or practices that start with the search criteria entered. Tip: Use this option if you are unsure of the spelling for a specific provider. You must enter at least two characters for this field to be valid. Example: If you type Sm in a Last Name field and select the Starts With option, your results will include records with last names Smith and Smyth, but not McSmyth. |
| Phonetic |
The Phonetic option tells the system to retrieve all providers or practices that sound like the search criteria entered. Tip: Use this option if you are unsure of the spelling for a specific provider. You must enter at least two characters for this field to be valid. Example: If you type Ph in a Last Name field and select the Phonetic option, your results will include last names beginning with the letter F as well as last names that begin with Ph. |
Provider Type and General Preferences Search Fields

Distance From Your Location Search Fields

If you need to find providers based on the location of their practice, enter the parts of the address on which you want to search. If you want to limit your search to providers who are located in a certain distance from your address, please enter the maximum distance you wish to travel in the 'Search within" field and then enter the address. You may also enter any address you desire, such as your work address, a relatives home address etc. You may combine the "Distance from your Location Search" with the "Provider Type and General Preference Search" if you desire. |

| Search Within | Providers located within a certain distance of the address provided. Indicates the maximum distance you are willing to travel to see a provider. Examples: 10 miles, 20 miles |
| Address |
An actual street address can be entered. It can be your street address, your work address, a relative's address or a provider's address. If an invalid address is entered, then NH MMIS uses the closes match to the entered address to perform the search. |
| City | The city in which the street address is located can be entered. |
| State |
The state where the street address is located can be entered. |
| Zip |
The zip code of the street address can be entered. |
Search Results Fields

| Doing Business As Name | Indicates the name the provider's business name. |
| Name |
Indicates the provider's name. If no address was entered when searched, this column displays the provider names in alphabetical order by default. Example: Dr. Mark Smith |
| Provider Type |
Indicates the provider's type of practice. Example: General Hospital; Osteopath, Individual; Physician, Group (MD) |
| Address |
Indicates the provider's street address. |
| Distance |
Indicates the distance from the location you entered in the Search fields (base address) and the provider's practice location. This column displays distances from closest to furthest, if address information is provided. Otherwise, Provider Name is sorted alphabetically. Examples: 5.6 miles, N/A |
